I'm a bit confused on the licensing of the four main services

Greetings,
I'm a bit confused on the licensing of the various two-way radio communications services.

As I currently understand them:

1) Ham radio = requires passing a test in order to obtain a license (the fee is nominal and insignificant).
2) CB radio = is governed by something called license by rule (not sure what that means exactly in plain English), and requires NO license, however it used to require a license when it first started back in the early post WWII war years
3) FRS = requires NO license
4) GMRS = kind of currently in a state of flux right now, thus it's somewhat hard to pin down, but from what I currently understand it went from requiring a $85 fee to now requiring only a $60 fee for a license... however, from what I've been learning from researching, the licensing of this service is largely being ignored and most people are just using it without a license at all. There was a petition filed to the FCC to do away with the licensing altogether but it fell through.
5) MURS = requires NO license
