New raised bed gardens

I'm building raised bed gardens for the first time this year and I'm wondering about depth of soil. I'm planning to build 3 5x12 beds plus lots of containers. The containers I've used before except for the potato bags so I know what my soil needs to be there but I'm unsure about the raised beds. Do you put straw down first then add soil? Add sand to the soil? I've read so many different things that I have information overload and would love some advice from people who have actually done it. Thanks for any information!!
