"Lost? Follow the river."

I've heard this a hundred times. If you're lost and in a survival situation, find a river and follow it to civilization (or a coast, which you then follow to civilization).

But is it really a reliable rule of thumb? Couldn't it potentially get you more lost, or stuck, or something? The river can squeeze through small rock openings you can't, and plummet from heights that would kill you. And should you follow it upstream or downstream?
