Sizing a vest

Want to buy my buddy a IIIA vest for Christmas
He currently has a gel vest that he says doesn’t fit properly, ?!!

Question: what is the proper way to size a vest?

His wife is in on it
Do I just have her get his T-shirt size and go one size larger? The instructions I’ve read say “one size larger”. Larger than what? ( his wife is in on it so snitching a shirt wouldn’t be soooo weird )
