Thinktank a strange delerium issue

my 66 year old friend had what they thought was a TIA about two weeks ago. Then they thought several others daily. Symptoms were non typical so the specialists(vascular specialist and physician specialist) have changed their tunes on it. She had mild slurring, mild unsteadiness, sometimes flushed face, and choppy gait increasing over 3 days but no left/right sided problems, headache or other neuro obs.

Ongoing symptoms are mild lethargy, slower response to talking, and slightly innattentive, some periods of full lucidity. No problem with memory.

Then these almost clockwork daily 'episodes' ( which they thought might be TIA's at first) which come on around late morning/lunch time. Start with mildly intoxicated like behaviour, some slurring, to mild disorientation and slight unsteadiness on feet.

They come on in 15-20 minutes, last about 2 hours, and then go back to mildly fluctuating symptoms. Sometimes a milder one at night too, or back and forth from symptoms to normal. They are also generally unaware they are exhibiting neuro symptoms except feeling unwell, stomach pain, nausea at times during them( sometimes nothing)

Everything has come back clear so far.
MRI brain, brain CT, CT angiogram, Holtzmann monitor, chest Xray, delerium screen, full blood work up, carotid doppler all clear. Some history of pancreatic inflammation and pain.

Symptoms are being called a case of fluctuating 'delirium' with unknown underlying mechanism at this stage. They are looking closer at pancreas.

She is a terrible snorer, had pretty bad reflux disease, a lot of stomach discomfort this year, may be missing sleep.

Other issues, on acid blockers, statin+ blood thinner( just since this incident) and a hypertensive med. This person is looking to take herself off ALL meds if nothing else works. Gutsy move but she is.

If anyone is still reading at this point, I congratulate you and feel free to offer any opinions or advice.

We are at a brick wall. The person has already spent 12 days in and out of imaging and hospitals. In our overloaded so called 'free health system', the neurologists are handling patient loads in the hundreds so their devotion to this issue is probably only cursory...
