Define "pre-existing" conditions please

What is your definition of a pre-existing condition exactly? I'm specifically asking the more left-leaning members of this forum, because I don't think anyone has agreed yet on what specifically that entails.

I can maybe understand including those born with a genetic defect or some other form of illness. For them, it's not like they ever had a chance for their condition to be non-existing. I still don't agree that they should receive health coverage at the same rate a healthy person would (after all, it's not MY fault you were born with it, them's just the breaks. I shouldn't have to pay for your bad luck), but I can concede that for the sake of human charity, helping the truly less fortunate, etc.

I could even understand someone who was never diagnosed with a condition and thus had no idea that they had it. As long as they had purchased and maintained coverage prior to their diagnosis. That's the whole point of insurance after all, right?

But what about someone who has not purchased health insurance, smoked for 20 years, been diagnosed with emphysema, then all of a sudden needs health coverage? Would that count as a pre-existing condition to liberals?

What about someone who gets pregnant, then wants to purchase health "insurance" to cover the pregnancy?

I guess my question is, where do people draw the line of what constitutes a pre-existing condition or not? All I hear from Congress is the phrase "pre-existing conditions" repeated ad nauseum, but I have yet to figure out exactly what they mean when they refer to it.

I think we as a nation need to DEFINE exactly what we are talking about before we can make laws about it.
